Newborn Screening in the UK: Heel Prick Test, NIPE and Baby Health Checks
Reviewed by Dr Bryan Beattie, Consultant in Fetal Medicine
Newborn screening in the UK helps detect serious conditions early, often before symptoms appear, allowing prompt treatment and improved long-term outcomes.
Consultant-led newborn screening guidance for families across Cardiff and the UK.
This guide explains the NHS newborn screening programme, including the heel prick test, the Newborn and Infant Physical Examination (NIPE), heart screening, and options for extended genetic and metabolic screening.
Why Is Newborn Screening Important?
Many serious conditions are not visible at birth. Screening allows early diagnosis before symptoms develop, helping to reduce complications and support healthier long-term outcomes.
- Early detection of serious conditions
- Prevention of complications
- Supports normal development
- Improves long-term health outcomes
What Newborn Screening Tests Are Offered in the UK?
1. Newborn Blood Spot Test (Heel Prick Test)
What is it?
A small blood sample taken from your baby’s heel to screen for rare but serious conditions.
When is it done?
Usually on day 5 after birth.
Conditions screened:
- Sickle Cell Disease
- Cystic Fibrosis
- Congenital Hypothyroidism
- Inherited metabolic conditions including:
- Phenylketonuria (PKU)
- MCADD
- Maple Syrup Urine Disease (MSUD)
- Glutaric Aciduria Type 1 (GA1)
- Isovaleric Acidaemia (IVA)
- Homocystinuria (HCU)
Results: Usually available within a few weeks. You will be contacted if further tests are needed.
2. Newborn and Infant Physical Examination (NIPE)
What is NIPE?
A detailed physical examination carried out by a trained healthcare professional.
When is it done?
Within 72 hours of birth and again at 6–8 weeks.
What does NIPE check?
- Heart abnormalities
- Developmental Dysplasia of the Hip (DDH)
- Eye conditions such as cataracts
- Undescended testicles (in boys)

Limitations of Hip Examination
Physical examination alone can miss some cases of hip dysplasia, particularly milder forms. This is especially relevant if risk factors are present, such as breech position, family history, or certain foot abnormalities.
Why Consider a Newborn Hip Ultrasound Scan?
A DDH hip ultrasound scan provides a more detailed assessment and can detect subtle abnormalities early.
- Non-invasive and safe
- More sensitive than examination alone
- Supports early treatment and better outcomes
3. Pulse Oximetry Screening (Heart Screening)
What is it?
A simple test measuring oxygen levels in your baby’s blood.
When is it done?
Usually within the first 24 hours after birth.
Why is it important?
It helps detect serious heart conditions that may not be obvious on examination alone.
Extended Newborn Screening (Genetic and Metabolic Testing)
Some conditions are not included in the NHS screening programme but may be detected through expanded testing.
- Spinal Muscular Atrophy (SMA)
- Duchenne Muscular Dystrophy (DMD)
- Additional rare genetic and metabolic disorders
How is it done?
Usually via a simple cheek swab or blood test.
Availability:
Not routinely available on the NHS. It is often accessed privately.
How Does UK Screening Compare Internationally?
- UK: Screens for 9 conditions
- Europe: Often 20–40 conditions
- USA: Over 30 conditions
How Can I Optimise My Baby’s Screening?
- Newborn DDH Hip Scan
- Extended Newborn Genetic Screening
- Genetic Carrier Screening (before or after pregnancy)
